Referrals & Admissions
For inpatient rehabilitatioin services, we are pleased to admit those individuals
who can benefit from three hours or more of therapy nearly every day.
Therefore, we require a referral by a physician or an evaluation by a
Casa Colina physician or nurse liaison prior to admitting a new patient.
All medical-surgical patients require a referral from their physician
for a scheduled surgery, intensive care or medical-surgical stay.

In most instances, you will need a physician referral to schedule an appointment.
You can typically be scheduled for an appointment within 2 to 3 days,
or immediately if your condition is urgent. In order to speed up the pre-admitting
process, we'll verify your insurance information and will make every
effort to contact you to discuss your coverage benefits prior to your

Transitional Living Center
We accept adults and adolescents between the ages of 18 to 59 with appropriate
diagnoses. Age waivers are available for clients that do not fall in the
appropriate age range on a case-by-case basis. We usually respond to each
referral within 24 hours to expedite the admission process. Referrals
are evaluated on a case-by-case basis and determined by individual need,
through interviews, and review of medical records. Our admission liaisons
will work with you, your family members, doctors and insurance providers

Children's Services Center
We accept referrals from a variety of funding and medical sources based
on a variety of areas of concern, including speech therapy, physical therapy,
occupational therapy, music therapy, early intervention and family support.
We require a prescription for occupational therapy, physical therapy and
speech and language therapy services. Regional center referrals require
authorizations. School districts require an IEP with a signed services

Adult Day Health Care Center
An application must be completed both by the individual's family and
primary care physician. Once this paperwork has been completed and returned
to ADHC, an intake appointment will be scheduled. The intake appointment
takes place at ADHC, and must be attended by the individual and his or
her caregivers. The paperwork and guidelines of the program are reviewed
at the intake appointment. Although individuals can self refer, their
application paperwork includes two forms that must be completed by the
primary care physician. The individual must also have a current TB test.

Diagnostic Imaging Center
A doctor's referral is required. A patient or his/her doctor may call
to arrange for an appointment. The doctor's office may also fax his/her
request, and we will follow up with the patient. Referrals for hospital
inpatients are faxed over by the hospital nursing staff. For assistance,

Casa Colina at Apple Valley and Casa Colina at Rancho Pino Verde
Individuals may be referred by a family member, case manager, funder, therapist,
or physician. We will obtain a brief history and request his or her records
to determine if this individual is eligible for admission. Individuals
and their loved ones are encouraged to visit the residence prior to admission.
