Needs Assessment Survey -- Help us plan for the future

In order to serve the community better, every three years Casa Colina Hospital conducts a Needs Assessment to learn more about the needs of people we serve.

One part of the assessment is a survey of people who are part of the community we serve, people who have disabilities or are at risk of having disabilities.  Our next survey will be taking place during the Spring and Summer of 2011 and we have added the option for people to take the survey on line.

The results of the survey will be posted on this website when they are available in early 2012. 

We invite you to take this survey and help us know more about the needs of people who can benefit from rehabilitation services.
