Senior Evaluation Program in Southern California

Gauging the Aging Process

"Does my father need more help?" "Should we consider a nursing home for our mother?" We understand what it's like to struggle with these questions. As people advance in age, it's important to strike a balance between respecting their independence and offering the right kind of assistance. Which is why we created the Outpatient Senior Evaluation Program for individuals over the age of 65 with mild to moderate impairments. It is without a doubt, one of the most thorough and compassionate ways to maintain senior health and well-being as well as determine the most appropriate level of assistance required by an aging individual.

We take a multidisciplinary approach to caring for the aging.

Our innovative program combines a physician-led team of clinical specialists in one convenient setting. The first appointment involves a pre-screening evaluation with a physical medicine and rehabilitation doctor who will determine appropriateness for admission into the program. A geriatric physician will then provide a comprehensive and thorough evaluation. Together, they will direct the clinical team who will conduct a series of on-site screenings designed to establish a problem list and suggestions to improve or maintain the highest level of function possible for the patient.

The assessment period requires a series of visits to Casa Colina's Outpatient Center over the course of 3 to 5 days. The focus will be on four assessment areas - functional ability, physical health, psychological health and socio-environmental health.

Our team of clinical specialists includes:

  • Physician who is board-certified in physical medicine and rehabilitation (physiatrist)
  • Physician who is board-certified in both geriatric and internal medicine
  • Physical, occupational and speech therapists
  • Neuropsychologist
  • Radiologist
  • Pharmacist
  • Dietician
  • Audiologist
  • Case manager /social worker
  • Other clinicians as required for the individual patient

After a complete assessment, our clinical team will meet with the patient, patient's family and/or caregivers to discuss the results and develop a recommended plan of care. So patients and their families will feel reassured about their decisions and plans for the future.

Outpatient Senior Evaluation Program patients will benefit from:

The Outpatient Senior Evaluation Program is recommended for patients who have (or had):

  • Arthritis
  • Cardiac disease or COPD/pulmonary problems
  • Change in mobility
  • Changes in behavior or mental status
  • Congenital disorders such as Spina bifida
  • Decline in memory or dementia/cognitive issues
  • Deconditioning
  • Decrease in ability to function independently
  • Dressing, grooming, or bathing difficulties
  • Eating or swallowing difficulties
  • Fractures of the femur or dislocation of the hip
  • Hearing or vision problems
  • Incontinence
  • Neurological diseases such as Guillain-Barre syndrome, Multiple Sclerosis, Muscular Dystrophy, Parkinson's disease, Cerebral Palsy, or post-polio syndrome
  • Stroke or brain injury
  • Weakness, unstable gait or balance or frequent falls
  • Wounds that won't heal

Our goal is to help improve the patient's quality of life now – and later.

Tools and resources will be provided to help keep the patient as functional as possible. We may recommend the patient return home supported by outpatient rehabilitation or the help of a family member. Or we may suggest home health, assisted living or skilled nursing care. In every instance, we will help determine the most appropriate level of care, so patients and their families will feel reassured about their decisions and plans for the future.
