Clients at the TLC experience significant gains in functional communication skills
, evidenced here with clinically relevant changes in memory and problem solving abilities between admission and discharge from TLC. Specifically, the mean score represents the level of assistance and time needed to either recall information or recognize and solve different tasks or problems. We observe clinically significant changes from maximum or moderate to a minimal level of assistance needed.

100% of clients who come to the TLC with problems swallowing demonstrate significant improvement.
To evaluate changes in an individual’s functional ability to intake food and liquid we used the functional oral intake scale. We observe functionally relevant changes from a limited oral diet supplemented with feed tubes to a total oral diet with only minimal limitations in the type of liquid or food.

100% of our clients exhibit a reduction in their need for caregiver supervision when they discharge from TLC.
Using the supervision rating scale, which was developed to measure the level of supervision that a patient receives from caregivers, we observe clinically meaningful changes from full-time direct to indirect, part-time or overnight supervision. This significant change in the level of supervision needed has the potential to acutely affect a patient’s independence, community re-integration, discharge location, and result in long-term savings for insurance companies.
