• 66% of clients are still living at home 12 months following discharge from the TLC
• 83% of clients have not been re-hospitalized since discharge from the TLC
• 83% of clients report that their physical abilities were maintained or improved when asked 12 months following discharge
• 100% of clients report communication abilities were the same or improved 12 months following discharge
• 100% of clients report emotional and behavioral control was the same or improved 12 months following discharge
• 50% of clients report that they have returned to work or school 12 months following discharge

And clients are satisfied with the care they recieved.

• 100% of clients (6/6) stated they would return to the Home & Community Program if they needed help again
