For more than 60 years, Casa Colina has been at the forefront of care for individuals recovering from stroke, offering a nationally renowned continuum of care that includes acute hospital rehabilitation, short-term residential rehabilitation and day treatment, home and community services, outpatient care, and adult day healthcare. Patients are admitted to the level of care most appropriate to their health condition, and services are personalized to meet their unique needs.

The optimal choice for inpatient rehabilitation

For some, stroke rehabilitation starts in our hospital’s Joint Commission-accredited, 68-bed Acute Rehabilitation Wing. Here, patients receive at least three hours of intensive therapy a minimum of five days per week from experienced therapists, many of whom possess certifications in neurological rehabilitation.

Our acute rehabilitation care provides the intensive therapy and medical treatment patients need to return to the highest-possible level of function. Led by board-certified physicians, our clinical teams hold weekly conferences to discuss patient progress, designing a detailed, customized clinical pathway of care for each patient’s rehabilitation and recovery after a stroke.

Transitional living to prepare for home

Casa Colina’s Transitional Living Center (TLC) is a 42-bed, CARF-accredited treatment facility offering clients recovering from stroke a bridge between inpatient rehabilitation and home. Residents undergo up to six hours of individual and group therapy, six days per week, to help them regain function and increase their independence.

All care at the TLC is directed by a physician specializing in physical medicine and rehabilitation, and our expansive care teams include physical therapists, occupational therapists, speech-language pathologists, neuropsychologists, recreational therapists, nursing staff, case managers, and other specialists as required. Clients can access a variety of services, including short-term residential rehabilitation, day treatment, and home and community care.

At the TLC, we focus on functional real-world tasks that prepare clients for challenges they will encounter at home, at work, or in their communities. Our daily programming takes clients on community outings to practice skills in real-world settings and improve their ability to transition back into their daily lives.

For families traveling from a distance to be near their loved one during rehabilitation, we offer on-site housing on a first come, first served basis.
