Transitional Care in Southern California
Preparing for What's Next After an Injury or Illness
When someone has suffered a traumatic brain injury, spinal cord injury
or other neurological trauma, the idea of returning home after being discharged
from the hospital may simply be too overwhelming. It can be difficult
to navigate around the house, communicate with loved ones or even get
ready for the day - let alone venture out into the workplace and community.
More time is needed to adjust to one's challenges after a serious
injury or illness. Which is why we created the Casa Colina Transitional
Living Center (TLC) to assist adults and adolescents with their post-acute
recovery. We offer both 24-hour short-term residential rehabilitation
and day treatment to suit the client's individual needs.
In this intensive, structured therapeutic environment, our therapy team
uses a variety of individual and group rehabilitative strategies to maximize
each patient's full potential. And to ensure standards of quality
are met, the Transitional Living Center is fully accredited by the Commission
on Accreditation of Rehabilitation Facilities (CARF).
