Community & Equity
Commitment to Health Equity at Casa Colina
At Casa Colina Hospital and Centers for Healthcare, we believe every person deserves exceptional, high-quality care, centered on helping each individual reach their fullest potential. Our commitment to health equity means ensuring that every member of our community has the same chance to achieve their best possible health — regardless of background or circumstance.
As part of that commitment, Casa Colina participates in the California Health & Human Services Agency’s Office of Health Care Access and Information (HCAI) Health Equity Reporting initiative. This report shares how we are working to identify and reduce disparities in care, patient outcomes, and experience for the many different populations we serve.
What is the HCAI Health Equity Report?
The HCAI Health Equity Report is a statewide effort designed to help hospitals better understand how social, economic, and demographic factors influence health outcomes. It includes data on race, ethnicity, language, and other important characteristics—highlighting where disparities may exist.
By collecting and sharing this information, Casa Colina and other hospitals across California can evaluate progress and implement meaningful strategies to ensure that every patient receives the highest quality of care- not matter who they are or where they come from.

Our ongoing commitment
We use insights from the HCAI Report, together with our Community Health Needs Assessments (CHNA) and internally collected data on Social Determinants of Health, to guide our quality-improvement efforts, program development, community outreach, and partnerships. Ultimately, our goal is to create a healthcare environment that is inclusive, responsive, and equitable for all.

Our Community Benefits Report and Community Healthcare Needs Assessment
Casa Colina Hospital and Centers for Healthcare is focused on providing direct medical and rehabilitation services in the community as stated in its mission: “to provide individuals the opportunity to optimize medical recovery and rehabilitation outcomes in a safe environment that respects their dignity, diversity, and individuality.” We are aware that, for some people, lack of insurance or other funding is a barrier to receiving healthcare. In addition, the health and well-being of the community as a whole, and the strength of our sister organizations in creating that network of care, are critical factors for all of us.
As a nonprofit 501(c)(3) organization, Casa Colina has a special responsibility to ensure that our services are available to all people who can benefit from them and that we do our part in providing for the health and wellness of the community.
In the Fiscal Year Ending (FYE) 2025, Casa Colina provided community benefits in the amount of $13.219M, including free sports injury screenings, public health education, support for other community organizations, training for the next generation of nurses, therapists, and physicians, and more.
These community benefits are prioritized based on the healthcare needs of people in our community. To learn more about what these specific needs are, every three years Casa Colina conducts a Community Health Needs Assessment. The most recent Community Health Needs Assessment was completed in February 2024, is available here, and was approved by our Board of Directors February 23, 2024. (The previous 2021 report is available here.)
The Implementation Strategy describes how Casa Colina plans to address significant health needs identified in the 2024 Community Health Needs Assessment. For each significant health need, Casa Colina provides programs and resources and collaborates with other hospitals and organizations in the community to address the needs of the people we serve. The most recent report was completed in February 2024, is available here, and was approved by our Board of Directors February 23, 2024. (The previous 2021 report is available here.)
To maintain nonprofit 501(c)3 status, Casa Colina completes and publishes a Community Benefits Report. The report for FYE 2025 is available here. (Board of Director approval for our Community Benefits Report and Implementation Strategy occurred July 31, 2025 and is available here.)
