
For inpatient rehabilitation services, we are pleased to admit those individuals who can benefit from three hours or more of therapy nearly every day. Therefore, we require a referral by a physician or an evaluation by a Casa Colina physician or nurse liaison prior to admitting a new patient. All medical-surgical patients require a referral from their physician for a scheduled surgery, intensive care, or medical-surgical stay.

Casa Colina's Outpatient Centers in Pomona and Azusa accept referrals for therapy services from a variety of medical professionals such as a physicians, dentists, podiatrists, and others authorized by their license to diagnose. Services are provided at a frequency determined by the referring medical provider, your therapist, and your individual needs. Your therapist will discuss how often services are needed at your initial evaluation.
In most instances, you will need a physician referral to schedule an appointment. You can typically be scheduled for an appointment within 2 to 3 days, or immediately if your condition is urgent. In order to speed up the pre-admitting process, we'll verify your insurance information and will make every effort to contact you to discuss your coverage benefits prior to your first appointment.
Please arrive 15 minutes before your appointment time to register at both our Pomona and Azusa outpatient locations. You'll need to bring your insurance card as well as the name and phone number of your physician.

We accept adults and adolescents between the ages of 18 and 59 with appropriate diagnoses. Age waivers are available for clients that do not fall in the appropriate age range on a case-by-case basis. We usually respond to each referral within 24 hours to expedite the admission process. Referrals are evaluated on a case-by-case basis and determined by individual need through interviews and review of medical records. Our admission liaisons will work with you, your family members, doctors, and insurance providers throughout the referral/admissions process. We accept referrals from a variety of funding and medical sources based on a variety of areas of concern, including speech therapy, physical therapy, occupational therapy, music therapy, early intervention, and family support. We require a prescription for occupational therapy, physical therapy, and speech and language therapy services. Regional center referrals require authorizations. A doctor's referral is required. A patient or their doctor may call to arrange for an appointment. The doctor's office may also fax their request, and we will follow up with the patient. Individuals may be referred by a family member, case manager, funder, therapist, or physician. We will obtain a brief history and request appropriate records to determine if this individual is eligible for admission. Individuals and their loved ones are encouraged to visit the residence prior to admission.
