We invite former patients to tell their story, build partnerships, and help us increase the quality of care at Casa Colina.
Casa Colina’s Patient and Family Advocacy Council (PFAC) gives patients and families the chance to educate our clinicians and administrators about real patient experiences in an effort to increase the overall quality of care at Casa Colina. By placing patients at the center of the care model, the PFAC creates a culture of collaboration and transparency, where patients and their loved ones are empowered to influence their medical care in a meaningful way.
To be eligible, you must have been a Casa Colina patient (or friend or family member of a Casa Colina patient) within the previous two years. Individuals must be at least 18 years of age and in stable health.
For more information, or to join the PFAC, please contact the
PFAC team at 909/596-7733, ext. 2220 or
